Booking and the referral process
If you are paying for your IVF and fertility treatment
Patients can refer themselves directly to the Assisted Conception Unit if they are funding their own treatment, you do not have to wait for a referral from your doctor. We have two funding types – self funded and consultant-led personalised care, see our funding pages for further details including the cost of IVF.
To book an appointment complete the infertility referral form and email it to email@example.com.

We have no waiting lists, simply call to make an appointment and you can usually be seen within a few weeks.
NHS IVF and other funded treatments
If you you would like to apply for NHS IVF and other treatment funding you will need to be referred to the Assisted Conception Unit through your GP or Hospital.

Please complete the forms on the link available to download and bring with you to your appointment. Based on the information on the registration questionnaire, your eligibility for NHS funding will be assessed. If you are eligible for NHS funding, we will apply for the funding on your behalf and inform you when the funding becomes available.
